LDD Rules & Contract

We have provided our daycare contract so that you may further understand what is required of you and your dog before enrolling in daycare. You will be asked to read and sign this contract at the time of your dog’s acceptance into our daycare program.
 

Lombard Doggy Daycare (LDD) agrees to exercise due and reasonable care in the handling of dogs, and in keeping the facility properly enclosed and sanitary. All dogs are cared for by LDD without liability on LDD behalf for loss or damage from disease, theft, fire, death, earthquake, escape, injury, or harm to persons, other dogs, or property by said dog(s), or for other unavoidable causes. 
 

I HAVE READ AND UNDERSTAND THE FOLLOWING:

  1. I hereby certify that my dog(s) is/are in good health and have not been ill with any communicable condition in the last 30 days.
     

  2. I further certify that my dog(s) have not harmed or shown any aggressive or threatening behavior towards any person or any other dog.
     

  3. I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are in the care of LDD and release LDD of any liability arising from my dog’s attendance and participation at the daycare and overnight care.
     

  4. I have been made aware that Canine Infection Respiratory Disease is highly contagious in a close environment. There are more than ten strains (ie. Kennel Cough). The symptoms are the same: fever, vomiting, coughing, pneumonia, nasal discharge, etc. The virus is airborne and transmitted through fluids. While Lombard Doggy Daycare’s cleanliness standards are comparable to Veterinary Hospitals, LDD is not responsible if my dog contracts this virus. My dog is at extra risk if they are a puppy, mature dog, or have a weak immune system.
     

  5. I recognize that there are inherent risks of illness or injury when animals are allowed to be in close contact with one another. Such risks include, but are not limited to, problems resulting from: bites and scratches during rough play, mild skin allergy, diarrhea, giardiasis, and canine cough (doggie colds).
     

  6. I further understand and agree that in admitting my dog(s) to LDD, the staff at LDD have relied on my representation that my dog(s) is/are in good health and have not harmed or show aggressive or threatening behavior towards any person or any other dog.
     

  7. I understand that my dog(s) will be playing in open areas with other dog(s) and accept that when dogs play in groups, they will get dirty, and nicks and scratches may occur, as well as any other inherent risks that are involved and I hereby release LDD of any liability;
     

  8. I further understand and agree that LDD staff and volunteers will not be liable for any or all problems which may develop.I further understand and agree that any problems which develop with my dog(s) will be treated as deemed by LDD staff and volunteers, at their sole discretion, and that I assume full financial responsibility for all expenses involved if I or LDD decides to obtain medical treatment. 
     

  9. I accept that if my dog causes any excessive damage to the facility that I could be asked to pay for repairs.
     

  10. I understand that if my dog is not spayed or neutered by 6 months of age, they will not be allowed to participate in daycare with other dogs, or they will have to stay home.
     

  11. I further understand that if my dog shows any signs of aggression towards other dogs, that their acceptance into LLD will be reevaluated. They will be given 1 chance, depending on the situation surrounding the aggression before they are forbidden to participate in LDD. 
     

  12. Human aggressive dogs, or dogs with fear issues, will not be allowed to stay at LLD.
     

  13. As a responsible pet owner, I promise to keep my pet(s) up to date on all vaccines, including Bordetella, Leptospirosis, Distemper and Rabies, and all other state required vaccines. It is required by LDD that you provide official updated records from a veterinarian before you are allowed to attend daycare.
     

  14. I understand that if I do not use these services for over one year my information, including dog records, enrollment form, and daycare contract, may be deleted. Please let us know if you plan on travelling for extended periods of time.
     

  15. We have surveillance cameras, but we do not share or provide any copy of these records to our clients or employees.
     

  16. LDD is not responsible for informing of due dates on vaccines nor will LDD provide vaccine records for any reason. Furthermore, when pet(s) vaccines are updated, a copy must be provided to LDD for our records to stay updated, otherwise LDD will assume that your pet(s) are not current on vaccines and will be asked not to attend daycare until 7 days after actual vaccines are administered.
     

  17. REFUNDS: LDD does not provide refunds for no-shows or unused daycare sessions.
     

  18. EMERGENCY: In the event of an emergency, LDD staff will first attempt to contact the owner, followed by an attempt to contact the emergency contact person listed. LDD may also directly contact the veterinarian listed, or any emergency veterinarian of LDD choice, if the circumstances are deemed such that immediate treatment is necessary. LDD staff retains sole discretion in emergency matters, without liability, and the owner of the dog agrees to promptly pay for all medical treatments received.


I certify that I have read and understand the policies of LDD and have signed a contract agreeing to the conditions and statements of this agreement.
